Transitioning From Pediatric to Adult Care

For children with sickle cell disease, making a smooth transition to adult sickle cell care helps prevent gaps in care that could lead to a pain crisis and complications. Pediatric patients have their last visit with their pediatric provider at our clinic, where they can tour the facility and meet our staff. This warm introduction to their new care team helps ease the transition, so our young adult patients feel more comfortable when they return.
